LTC-RN-MDS Evaluator

Spring Creek Healthcare is seeking a part-time RN- MDS Evaluator. This part time position will be Monday- Friday for a total of 16 hours a week (actual dates and times to be determined). This position is eligible for Dental, Vision, Retirement and Paid Time Off.

Job Description:

The MDS Coordinator has the responsibility/accountability for the completion of the minimum data set by the Interdisciplinary Team for all long-term care residents. Is responsible for initiating the plan of care resulting from the MDS and for all long-term care evaluations and may assist in coordination of admissions to Long Term Care. Oversees MDS Evaluators and responsible for transmission of MDS data to State. Supports hospital and nursing policies. Understands disease processes and treatments as well as preventive and maintenance care for all age groups, but works predominantly with adults from ages 18 to expiration.

Minimum Work Experience:

  • Minimum of two years prior experience in Long Term Care setting with MDS experience. 

Minimum Education:

  • Graduate of an accredited RN program
  • Registred with Kentucky Board of Nursing or compact states

Required Skills:

  • BLS certification maintained
